Dr. Kordonowy’s Updated Concierge Plans

As a concierge doctor, Dr. Kordonowy has a direct relationship with his patients. What does this mean? In his practice, Dr. Kordonowy offers personalized and unlimited access for his patients. He offers four different concierge memberships: basic, gold, premier, and platinum. He has offered these services for a few years now, but the requirements have changed.

Dr. Kordonowy’s office, Internal Medicine, Lipid & Wellness, is now requiring all new primary care patients to be a member of one of his membership plans. Dr. Kordonowy is happy to see any patient for consultation for his single consultation fee.  Any patient wishing to establish for his primary and concierge care will be required to choose a membership option.

For services and items not included in the membership, members still have the option to pay cash prices.  These services and items include (but not limited to):

Another change that has occurred is the age requirements for each membership plan. These yearly plans are as followed:

Basic: Limited to ages 16-44 years of age.

Gold: Required for ages 45-64 years of age.  Younger clients may chose this option.

Premier: Option for All Ages and Required for Medicare
Platinum: Available to all clients- the most comprehensive program.

Basic – The Basic concierge health plan costs $49 per month with a one-time enrollment fee of $150. With this service, the patient receives:

• 3 office visits per year including a full physical examination annually.
• Annual ECG and body fat % analysis.
• Annual physical and comprehensive lab profile (retail price for this exceeds for 150 dollars.
• Same day appointments once established.
• Online access to medical records/results via a patient portal (private e-mail attached to your chart).
• Online messaging with Dr. Kordonowy and nurses via the e-mail portal.
• Free after hour phone consults- my normal after hours call fee is waived for members.
• Remote monitoring link
• Membership access to our cost-effective prescription generic dispensing program-potential savings of hundreds of dollars.
• Annual flu vaccine.

Gold – The Gold concierge health plan costs $89 per month with a one-time enrollment fee of $150. With this service, the patient receives:

• Unlimited office visits.
• The unique Inpatient Advocate Service (daily advocacy and personal visits when in the hospital- additional hourly charge applies).
• Annual physical and comprehensive lab profile.
• Annual ECG.
• Same day appointments once established.
• Online access to medical records/results via the patient e-mail portal.
• Online messaging with Dr. Kordonowy and nurses via the patient e-mail portal.
• Free after hour phone consults.
• Body fat % analysis, biophotonic anti-oxidant score and remote monitoring capability.
• Membership access to our cost-effective prescription generic dispensing program-potential savings of hundreds of dollars.
• Annual flu vaccine.
• Blood draw fees waived.

Premier – This plan costs $2,000 per year with no enrollment fee. With this service, the patient receives:

• 24/7 phone access to Dr. Kordonowy.
• Same day appointments for established patients.
• Online access to medical records/results.
• Online messaging with Dr. Kordonowy and nurses via the e-mail portal.
• Free after hour phone consults.
• Annual flu vaccine and other routine vaccines.
• Body fat analysis, antioxidant biophotonic score annually.
• Membership access to our cost-effective prescription generic dispensing program-potential savings of hundreds of dollars.
• The unique Inpatient Advocate Service (daily advocacy and personal visits when in the hospital- no additional hourly charge).

Platinum – The Platinum concierge health plan costs $2,500 per year with no enrollment fee. With this service, the patient receives:

• The unique Inpatient Advocate Service (daily hospital advocacy and personal visits when hospitalized- no additional hourly charge).
• 24/7 direct phone access to Dr. Kordonowy’s private cell phone.
• Same day appointments.
• Unlimited office visits.
• Annual physical and comprehensive lab profile.
• Same day appointments.
• Annual seasonal flu vaccine and other routine vaccines.
